All specimens must be accompanied by a Hematopathology Requisition.

From within Stanford Hospital and Clinics or Louise Packard Children's Hospital, contact your clinic nurse manager or call 723-6111 if you need a requisition
From outside, please call 650-736-4207 for instructions for submission
